Office staff is available Monday to Thursday from 8:00 AM to 5:00 PM and Friday from 8:00 AM to 4:00 PM.

Appointments for consultations and surgeries are scheduled Tuesday and Thursday from 8:00 AM to 5:00 PM and Friday from 8:00 AM to 1:00 PM.

Payment is due at the time of service. 

As a courtesy to our insured patients, we will contact your insurance company to determine as close as possible what will be your co-pay and we will submit the claim for you. Your co-pay is due at the time of service.  

All charges incurred are your responsibility regardless of the insurance coverage. Our relationship is with you, and not your insurance company.  

We do not accept Medicare and Medicaid.

We request a 24 hr. notice for any appointment that needs to be cancelled or rescheduled. We are happy to reschedule your appointment, but if no prior notification was made then payment is needed before we will reschedule.

We accept cash, credit card, debit cards, cashier checks, and Care Credit (Care Credit is a financing option. carecredit.com or 800-365-8295). We do not accept personal checks.
